Sunday Bulletin Announcement Use this form to submit an article or announcement for the Sunday bulletin. These must be submitted by Wednesday at 2:00 pm.
Submit Newsletter Articles Use this form to submit an article for the Good Shepherd Newsletter (Shepherd's Staff). Articles must be submitted by the 10th of the month.
Estimate of Giving Use this form to provide an estimate of your giving for the year.
Thrivent Lutheran Simply Giving© Program Use this form to authorize a regular gift to Good Shepherd or your favorite Lutheran institution or to pay tuition to a school that is part of the simply giving network.
Payment Voucher Use this form to request a reimbursement for payments or purchase made on behalf of Good Shepherd Lutheran Church.
Gifts Use this form to identify your gifts from a list of abilities, interests, experiences, passion and personal styles and indicate your interest in participating in ministries.
GSLC Scholarship Award Application Use this form to apply for a scholarship.
Infant Baptisim Use the information to provide information about the baptism of your child.
Emergency Consent This Emergency Consent Form will be used as authorization for treatment if a parent or guardian cannot be reached or cannot be with a child when such urgent care is needed
Child Safe Screening This form contains a summary of our child safe policy and background questions for those who wish to work with children's programs.
Picture Opt-out Occasionally, images of Good Shepherd events will be posted on the website. Complete this form to request that a picture that includes you or anyone else listed on the form will not appear on the website.
Picture Release Use this form to authorize the church to use your picture in printed material or the website.
